New-to-district students

Students new to the Anchorage School District:
To complete registration, new-to-district students will need:
- Current official record of immunization for diphtheria, pertussis and tetanus – also known as D-P-T – and polio, measles, mumps, rubella, hepatitis A, hepatitis B and varicella on or before August 19.
All children attending a preschool are required to register and be immunized against Haemophilus influenzae type b (Hib) and varicella or have a history of varicella disease (chicken pox). Students will be allowed to select courses but will not be allowed to attend classes until the immunization record is on file in the school. Tetanus-diphtheria (Td) boosters must be current (within the last 10 years).
- Birth certificate for kindergarten and first-grade students.
- Kindergarten students need to present a physical examination statement completed by a licensed physician, physician’s assistant, or state authorized advanced nurse practitioner within 12 months prior to entry into kindergarten or within 90 days after entry into kindergarten.
- All students transferring into the district will receive a health screening by a school nurse within 90 days, to meet the physical examination requirement.

Se le permitirá a los estudiantes seleccionar cursos pero no podrán asistir a clases hasta que el registro de inmunización esté en el archivo en la escuela. Los impulsores de tétanos- difteria (Td) deben estar al corriente (en los últimos 10 años).

Los estudiantes de jardín de niños necesitan presentar una declaración de examinación física llenada por médico con licencia, asistente de medico, o practicante de enfermería avanzada autorizado por el estado antes de 12 meses de la entrada a jardín de niños o durante los 90 días posteriores a la entrada al jardín de niños.

Todos los estudiantes que sean transferidos al distrito recibirán un monitoreo de salud por la enfermera de la escuela en los siguientes 90 días, para cumplir con los requerimientos de examinación física.

Immunizations
Parents must take official shot records to school registration for new-to-district students and to update school health records if new shots were given during the summer. Immunization exemptions may be granted for medical or religious reasons.
The number of vaccine doses for each required immunization varies depending upon a child’s age, grade and prior vaccination history. Please take your child’s shot records with you to determine if vaccination is needed. Childhood vaccines are available at physician offices, clinics, and public health centers throughout the community. Immunizations are available at the following locations at no cost.
