Activity Logs
Choose the right one for your needs!
Children, with parental supervision, record their daily physical activity during a one-month period. Completed Activity Logs are turned in to participating Healthy Futures school administrators. Fun incentives encourage youth participation.
The fitness logs are more comprehensive and include veggies/fruits eaten, glasses of water drunk and hours of sleep. Guidelines for how to use the Activity/Fitness Log (PDF) updated document attached.

How to join the Healthy Futures Team
- Keep YOUR very own Activity
Log with you and fill
it in daily
- Make sure you show it to your mom and dad!
- Each month turn your log into your Physical Education teacher
- A Healthy Futures Nalgene water bottle is yours when you complete
and turn in your first Activity Log!
