School Immunizations
Alaska elementary students need varicella (chickenpox) vaccination by July 1, 2009
The state of Alaska is requiring all children through grade six to have the varicella vaccination by July 1, 2009. ASD students must present their updated immunization record during school registration in order to enroll in summer school or for the 2009-10 school year. Read more... (PDF) |

Students with no shot records or waiting for shot records:

To be admitted, a child must receive one dose each of DTP/DT/Td, Polio, Hepatitis A, Hepatitis B, Varicella and MMR vaccine. Students in Kindergarten through Sixth Grade must have Varicella, as of July 1, 2009. A student with a history of Varicella disease must have written verification from a health care provider. Verification forms can be obtained from the school nurse. Preschool children must have Hib vaccine and varicella vaccine or have a history of varicella disease with documentation from their MD, DO, ANP or PA.

A signed physician's statement is required, if there are medical reasons a child cannot be vaccinated. Only a physician, physician’s assistant or advanced nurse practitioner licensed in Alaska is authorized to exempt children from immunizations.

To remain in school a medically verified record must be provided showing the dates of all required immunizations, or the remaining immunizations must be given.
- Children not complying with immunization requirements must be excluded from school.

Notifying the Anchorage School District
Once your child's shot record has been updated, be sure the district gets a copy. Send or fax a copy to our Health Services Program and bring a copy to your school when you register your child.

Obtaining a copy of the shot record or TB skin test
When a student is transferring within the district, the parent/guardian should ask the school nurse. They will be asked to fill out a records request, and then the nurse can print out the needed information. Administrative assistants and school secretaries also have the ability to print these records.
For a student who has graduated from or left the district, a request needs to be made to the Student Records department located on Hillcrest Drive next to West High School. The telephone number is (907) 742-4607.
Current elementary, middle, and high school student shot records can be obtained at the school. Parents need to fill out a request for records, submit it to the school nurse, and she will then print out the information and give it to the parents. Administrative assistants and school secretaries also have the ability to print out these records.
