To use these forms
- After you have downloaded and installed Acrobat Reader, click on the
link for the appropriate form. You may be given the opportunity to open
the file from its current location or to save the file to disk. For
now, open the file from its current location. The form will load in
your browser.
- After the form has loaded in your browser, you may type in all the
information required except your signature.
- After all your information is entered, you may then print the document
and sign it and get the other required signatures. (You may save the data in
your form to your computer if you have the full version of Acrobat.
Under the "File" menu, select "Save" and go through
the normal steps to save a document.)
